Weight Loss Challenge Pre-Visit-Questionnaire

Welcome to the Pershing Oaks Weight Loss Challenge! We hope to assist you in your journey to better health for your pet. We look forward to your consultation where we will evaluate your pet’s nutrition and activity to prepare a personalized plan for success. Please complete this questionnaire before your appointment, so our doctors will have time to review it before they meet with you.

Pet's Name:

1. What brand, flavor and type (dry kibble, canned, fresh) of food are you currently feeding your pet?

2. How much of this food are you feeding your pet and how many times a day? Please be as specific as possible – if you use a scoop for dry food, please measure the amount of food in the scoop with a measuring cup, if you feed ½ a can of wet food, what size (in ounces) is the can.

3. What brand, flavor, size and any other description of treats are you feeding your pet? Please list all treats given.

4. How many treats are you giving your pet per day? Per week? Please list the amounts of each treat.

5. Does your pet ever eat human food or table scraps? If yes, what human food are they eating, how much and how often?

6. Please describe your pet’s physical activities. How often are they walked? For how long? Do they have play time in the yard?

7. For cats: Do your cats go outside? Do they roam freely or are they on an enclosed patio?
